    Important Note:

    If you experience problem with submitting this form because of your email address, please follow the below instructions to resolve it:

    1. Click request removal 
    2. Enter your email address in the box and click Submit
    3. Submit the iCard form again

    After you have been officially registered in an Illinois course, please fill in the form below to request an I-card. Duplicate cards will not be issued.  If you do not receive your initial card, please contact our office. 

    The photoless I-card, your University of Illinois identification card, allows you to borrow material from the UIUC library and the statewide I-Share member libraries.
